Others think of an ultrasound if something feels unusual. A few assume MRI is the most advanced screening option. And many feel that without a family history, screening can safely wait.<\/p>\n<p>It sounds responsible, proactive and seems like the right approach.<\/p>\n<p>But here is the uncomfortable truth.<\/p>\n<p><strong>Far too many women who believe they are screening correctly are actually doing it wrong.<\/strong><\/p>\n<p>Not because they neglect their health, but because breast cancer screening is widely misunderstood. Different tests serve different purposes. When the wrong test is used, early cancers can easily be missed, leading to serious consequences.<\/p>\n<p>Understanding which method is best suited for routine breast screening can save your life.<\/p>\n<p><em>A story I hear too often<\/em><\/p>\n<p>A woman in her mid-forties once told me something I hear quite frequently.<\/p>\n<p>\u201cI check myself every month,\u201d she said confidently. \u201cIf something was wrong, I would feel it.\u201d<\/p>\n<p>She had never had a mammogram.<\/p>\n<p>When she finally came in for screening, the mammogram picked up a very small cancer deep inside the breast. It was far too small to feel by hand and caused no symptoms at all.<\/p>\n<p>Fortunately, it was detected early and treated successfully.<\/p>\n<p>What she said later stayed with me:<\/p>\n<p>\u201cI thought I was screening myself right. I didn\u2019t realise I was missing the one test that mattered most.\u201d<\/p>\n<p>Many women understand the importance of breast health, but their approach to screening may not be the most effective.<\/p>\n<p><em>Understanding the different breast screening tools<\/em><\/p>\n<p>To know why this happens, let\u2019s look at the tests women commonly rely on. Each one has a role, but they are not interchangeable.<\/p>\n<p><em>Self-breast examination<\/em><\/p>\n<p>Self-breast exams help women identify visible changes such as swelling, dimpling of the skin, or a large, obvious lump.<\/p>\n<p>However, self-exams have an important limitation: they can only detect lumps that are already large enough to be felt. Early cancers are often much smaller than that.<\/p>\n<p>Self-checks, therefore, <b>cannot replace imaging-based screening.<\/b><a href=\"#_ftn1\" name=\"_ftnref1\"><sup>[1]<\/sup><\/a><\/p>\n<p><em>Ultrasound<\/em><\/p>\n<p>Although ultrasound is widely used for pregnancy and other medical evaluations, it is not designed to be the primary screening method for breast cancer.<\/p>\n<p>Certain early signs of breast cancer, such as tiny calcifications, are difficult for an ultrasound to detect. Hence, it is not suitable for routine screening and early detection.<\/p>\n<p>Also, ultrasound results can vary between operators because the test depends heavily on individual skill and is not fully standardised <a href=\"#_ftn2\" name=\"_ftnref2\"><sup>[2]<\/sup><\/a><\/p>\n<p>Ultrasound is typically used as a <strong>follow-up diagnostic tool <\/strong>after screening mammography or when evaluating a particular concern.<a href=\"#_ftn3\" name=\"_ftnref3\"><sup>[3]<\/sup><\/a><\/p>\n<p><em>MRI<\/em><\/p>\n<p>MRI is expensive, requires an IV injection, takes longer to perform, can be claustrophobic for some patients, and can sometimes mistake harmless structures in the breast for cancer. This can lead to unnecessary anxiety, additional testing, and extra costs.<sup>2<\/sup><\/p>\n<p>However, because of its high sensitivity, MRI may be recommended for women at very high risk of breast cancer, such as those with a strong genetic predisposition.<\/p>\n<p>But MRI is not ideal for routine screening for most women.<\/p>\n<p><em>Why mammography remains the gold standard<\/em><\/p>\n<p>When it comes to detecting breast cancer early, mammography remains the most reliable screening tool available, recommended by reputable organisations worldwide, such as the CDC (USA) and Breast Imaging Society of India (BISI).<\/p>\n<p>Mammography uses low-dose X-rays to look inside the breast and detect very small changes in tissue. These changes can appear long before a lump forms.<\/p>\n<p>Studies show mammography can detect breast cancer with <b>high accuracy, often between 89 and 97 per cent<\/b>.<a href=\"#_ftn4\" name=\"_ftnref4\"><sup>[4]<\/sup><\/a><\/p>\n<p>More importantly, mammography is the <b>only screening method proven to reduce deaths from breast cancer<\/b> because it finds the disease early when treatment is simpler and more effective.<a href=\"#_ftn5\" name=\"_ftnref5\"><sup>[5]<\/sup><\/a><\/p>\n<p><em>How 3D mammography is improving early detection<\/em><\/p>\n<p>Technology in mammography has advanced significantly over the years.<\/p>\n<p>Traditional 2D mammography creates two flat images of the breast. While effective, overlapping breast tissue can sometimes make interpretation challenging.<\/p>\n<p>Newer <b>3D mammography<\/b> machines capture multiple images of the breast from different angles, allowing radiologists to examine breast tissue layer by layer. The result is greater clarity and improved detection.<\/p>\n<p>Studies show that <b>3D mammography can detect 20 to 65 per cent more invasive cancers compared with traditional 2D mammography alone<\/b>.<a href=\"#_ftn6\" name=\"_ftnref6\"><sup>[6]<\/sup><\/a><\/p>\n<p>This improvement is particularly important for women with dense breast tissue, which is common among Indian women in their forties.<\/p>\n<p><em>How AI is helping 3D mammography<\/em><\/p>\n<p>When artificial intelligence (AI) systems are trained using millions of mammography images, they help radiologists identify subtle patterns that may indicate early cancer.<\/p>\n<p>AI does not replace doctors. Instead, it acts as an additional set of expert eyes and helps screen more patients than before due to its operating speed.<\/p>\n<p>Studies now show that AI-supported 3D mammography further improves early cancer detection, reduces missed findings, and increases reading accuracy.<a href=\"#_ftn7\" name=\"_ftnref7\"><sup><sup>[7]<\/sup><\/sup><\/a><\/p>\n<p><em>When and how often should you screen?<\/em><\/p>\n<p>Doctors recommend that women with average risk begin <b>annual mammography screening at age 40<\/b>.<\/p>\n<p>Women with higher risk factors \u2013 such as a family history of breast cancer, genetic risk, dense breasts, late motherhood, or limited breastfeeding \u2013 may be advised to begin screening earlier.<\/p>\n<p>The best approach is to discuss your personal risk with your doctor and create a screening plan that works for you.<\/p>\n<p>Consistency matters. One screening event cannot protect you for life. Lifelong screening at a doctor-prescribed frequency is important to detect even the smallest changes early.<\/p>\n<p><em>In conclusion<\/em><\/p>\n<p>Breast cancer screening is not about doing something convenient when possible. It is about doing <b>the right test at the right time<\/b>.<\/p>\n<p>Self-exams build awareness. Ultrasound and MRI have important roles in specific contexts. But when it comes to routine screening for early detection of breast cancer, <b>mammography remains the foundation of screening<\/b>.<\/p>\n<p>Advances such as <b>3D imaging and AI-assisted analysis<\/b> are making mammography even more accurate and reliable.<\/p>\n<p>If you haven\u2019t been using mammography, it may be time to rethink your approach.<\/p>\n<p>Talk to your doctor. Look for a reputable screening centre that offers modern mammography, preferably 3D.<\/p>\n<p>Make mammography a regular habit.<\/p>\n<p>Because when breast cancer is found early, treatment becomes easier, outcomes improve, costs are reduced, and lives are saved. There is no way to prevent breast cancer.
